• Title, author, edition, and copyright year of our book
• An exact copy of the material you wish to use
• The name and address of your publisher
• Title of your Work and the context in which the material will be used
• Format of your Work: hardcover, paperback, E-book, CD-ROM, etc.
• Language of publication
• Geographic territory of distribution (United States, North America, throughout the world, etc.)
• Projected publication date of your work
• Estimated number of copies to be printed over the life of the edition
• Your full name, complete mailing address, phone, fax number, and e-mail
